Registered Clinical Dietitian
Registered, Licensed Dietitian & Diabetes Education Quality Coordinator

JOB SUMMARY: Registered Dietitian is responsible for assessing and educating patients, residents, and staff. Person serves a dual function of meeting acute and long-term care nutrition needs. Position works closely with Certified Dietary Manager and dietary staff to produce quality meals. Position oversees and participates in Diabetes Education and outpatient Medical Nutrition Therapy.

ESSENTIAL DUTIES AND RESPONSIBIILITIES:

1. Perform nutrition assessment for long term care, acute care, swing bed, surgical patients.
2. Develop patient/resident menus in collaboration with other team members.
3. Identify, monitor, and evaluate nutrition support regimens.
4. Assess patients’ learning needs, barriers to learning, preferred learning styles and education level and provides nutrition education accordingly.
5. Identify potential food-drug interactions and provides education based on identified need.
6. Facilitate resources throughout the continuum of care to foster patient outcomes.
7. Perform outpatient and inpatient consultations as ordered by the provider.
8. Collaborate with the patient and/or family member, and health care team to assess nutrition and education needs of patients.
9. Provide services and establish goals based on the identified needs of patients and/or family members.
10. Perform MDS (Minimum Data Set) assessments for long term care.
11. Quarterly charting on long term care center residents and monthly charting as needed for monitoring of high nutritional risk (wounds, weight changes)
12. Attend care conferences for long term care residents. Update resident care plan as needed.
13. Provide community educational opportunities.
14. Participate in interdisciplinary and departmental quality improvement criteria for patients across the continuum of care through quality goals and by supporting the facility goals.
15. Provide care according to organizational ethics, licensure, and the Standards of Professional Practice for Dietetics Professionals.
16. Communicate sound scientific principles, research, and theory to health care team members in optimally managing patients’ care.
17. Foster a positive and collaborative working relationship with dietary CDM and staff.
18. Participate in Discharge Planning Team, community diabetes support group, and other committees as assigned by the Department Director.
19. Completion of other duties as assigned by the Department Director.
20. Fulfill the role of Diabetes Education Quality Coordinator including:
1. Oversight of DSMES Services: Planning, implementing, and evaluating DSMES services using the ADA Program Recognition Standards
2. Participating in and ensuring annual review of ongoing DSMES-specific continuous quality improvement plan
3. Supervision of Educators: developing training materials, participates in peer review process and appraises performance of Diabetes Educators (including self) to identify areas of improvement and develop an improvement plan
4. Ensures maintenance of 15 hours of continuing education annually for self and Diabetes Educators specific to diabetes, diabetes related topics, behavior change, and self-management strategies
5. Educates patients with diabetes using evidence based DSMES curriculum and provides Medical Nutrition Therapy for management of diabetes and other health conditions.

COMPLEXITY & DIFFICULTY:
• Must work collaboratively with the dietary staff and CDM to create meals which tastes good, meets correct temperature specifications and look appealing.
• Understanding resident and patient food preferences when designing meals
• Understanding Long Term Care regulations regarding cultural competency and food choices

MINIMUM QUALIFICATIONS
Must hold a minimum of a Bachelor’s degree in Nutrition or Dietetics and have completed a Dietetic Internship from an accredited institution or a Coordinated Undergraduate Program.

Certificates, Licenses, Registrations
• Bachelor’s or Master’s degree in Nutrition or Dietetics from a ACEND accredited program
• Registered with Commission on Dietetic Registration, or registration eligible, to be obtained within three months of hire.
• Current license or ability to obtain license through the Wyoming Dietetics Licensing Board
